4. Children First: National Guidelines for the Protection and Welfare of Children 1999 ; should be fully implemented on a national level. Organisations and all those who work with children need to be aware of and utilise these guidelines so that identifying and reporting child abuse is dealt with as efficiently and effectively as possible. Organisations, including childcare providers should develop internal child protection policies, in conjunction with the Children First Officer ensuring staff and volunteers understand how the reporting process works. Providing ongoing supervision and training of personnel is also critical in improving professional practice in all statutory, voluntary, community and private settings. The Minister for Children has shown commitment and dedication in securing the expansion of the vetting service and there is clearly a commitment at the highest levels to further improve child protection systems. However, Irish society needs to maintain its commitment to building a strong and sustainable child protection system. It is therefore imperative that the further developments needed are implemented as soon as possible, as delays in directly addressing this area simply places young people at unnecessary risk. We must all continue to campaign for the introduction of a comprehensive vetting system until a strong and sustainable child protection system is in place. There are isolated reports of adverse effects, mainly hepatotoxic reactions, associated with the use of single-ingredient and combination products containing valerian. However, it is difficult to establish causal relationships in these cases because other factors such as concomitant medication and the ingestion of other herbal ingredients were involved.2 The Medical Toxicology Unit for Guy's and St Thomas' NHS Trust, which handled several of these reports, has suggested that in some cases reactions may have been idiosyncratic. Some studies suggest that valerian may cause a slight impairment of performance of skilled tasks for a few hours following ingestion. However, "hangover" effects impairment of performance the morning after valerian treatment ; do not appear to be a concern. The EMEA HMPWG proposed core SPC for valerian states that "intake of valerian preparations immediately up to two hours ; before driving a car or operating machinery is not recommended" and that the effect of valerian preparations may be enhanced by consumption of alcohol.8 There are no reported drug interactions for valerian, although potentially valerian and other herbs with CNS depressant activity ; may potentiate the effects of concomitant sedative medicines. Drug-herb interactions will be considered in detail in a later article. In vitro cytotoxicity and mutagenicity have been documented for valepotriates, although the clinical significance of this is unclear given that these compounds are unstable and unlikely to be present in finished products.2 These and other data relating to the safety of valerian have been reviewed.2, 3, 5, 11, Safety data for hops, lemon balm, passionflower, lavender and Jamaica dogwood are generally lacking. Limited adverse event data are available from clinical trials of combination preparations of valerian with hops and lemon balm.2 Pregnancy and lactation There is a lack of data on the safety of valerian taken during pregnancy and lactation and, therefore, use should be avoided as a general precaution.2, 8 The benefits of other herbs traditionally used in insomnia have not been clearly established and, because there is a lack of data on their safety during pregnancy and lactation, as a general precaution, use should be avoided.

This paper provided an updated description of the gastric cancer burden by region in Italy, in terms of time trends through 2010 and point estimates in 2005. Estimates in 2005 are reliable from a methodological point of view and they are informative for an updated description of the burden of this disease. The estimates rely on the cause-specific mortality statistics and on the relative survival observations from the Italian cancer registries, whose data quality is reliable. The observed incidence data from local cancer registries and the estimated regional incidence matched very well over the entire period covered by cancer registration. The results of the validation procedure are illustrated in detail elsewhere18. An additional validation is the close matching between the national estimates and the values obtained by pooling the results of all the regional estimates. The reduction of the stomach cancer incidence from 1970 to 2010 is considerable, about 60%, and is mostly concentrated in the first 2 decades, whereas it slows down after the 1990s. The incidence and mortality among men are about twice as high as the rates for women all over the country, and they decrease with the same slope. Disease occurrence declined more markedly in the areas at higher risk, thus more homogeneous in and vinblastine.
